About the role
- Job Requirements: Valid RN license with the state of Pennsylvania OR LCSW degree 3+ years experience as a clinical care coordinator, liaison, or case manager preferred Self motivated with excellent time management and communication skills Must have clinical knowledge of disease entities common to Home Health and Hospice Must have a valid Driver's License
