About the role

Overview As a Pharmacist, you are the trusted expert patients rely on to understand their medications and use them safely and effectively.
You help make life better by ensuring patients are informed and confident in their drug therapy-playing a vital role in their overall care and well-being.
CLINICAL DUTIES: Optimal patient care outcomes from drug therapy are ensured through clinical pharmacy activities.

Perform regular re - evaluation of medication therapy to assess whether therapeutic goals are achieved and to proactively intervene on medication related issues.
Perform medication therapy management and monitoring based on Legacy specific Collaborative Practice Protocols (CPP).
Document interventions, recommendations, and protocol driven changes in therapy in the EHR.
Monitor, document, report, and manage adverse drug events.

As required by licensure.
Licensure Current Registered Pharmacist License (RPh) for applicable state required.
BLS for Provider certification from the American Heart Association required within 3 months.
Pay Range USD $82.40 - USD $95.58 /Hr.

Demonstrate clinical competence by completing the clinical competency modules assigned in the electronic learning system for the pharmacist role.
Ensure that a patient's medication therapy is consistent with the Legacy or health insurance formulary and medication use guidelines and policies.
Ensure that patients are assessed comprehensively in a standardized way using electronic health record (EHR) tools.
Provide accurate and timely medication information to health care staff and patients.
