About the role
- This role helps connect individuals to essential healthcare and resources, acting as a liaison between the community and the care team to ensure access to necessary services.
Responsibilities
- Offer case management support, including scheduling appointments, tracking referrals, and providing follow-up care.
- Use personal experiences and cultural knowledge to build rapport with patients, promoting engagement in care and trust in the program.
- Maintain accurate records of patient encounters and services provided, ensuring compliance with program protocols and HIPAA regulations.
- Support safety protocols for the team during outreach activities and assist in managing emergent situations as needed.
Requirements
- Ability to stand, walk, and move for extended periods while providing care in various environments, including encampments, shelters, and mobile units.
- Comfort working outdoors in varying weather conditions and navigating uneven terrain.
- Ability to bend, stoop, and kneel as needed during patient care or outreach activities.
- Bilingual (English/Spanish) Required.
- Strong interpersonal and communication skills with the ability to navigate complex situations effectively.
- Lived experience or significant experience working with underserved populations.
- Familiarity with community resources, public health programs, and social services in the area.
- Valid driver's license with a clean driving record and comfort driving in urban environments.
- Passion for community health, patient support, and improving access to healthcare services.
- Physical Requirements:
- Capacity to lift, carry, and transport equipment or supplies weighing up to 30 pounds.
- Adequate manual dexterity to perform medical procedures, operate equipment, and manage electronic health records on portable devices.
- Visual and auditory acuity to perform essential functions such as patient assessments and effective communication.
Nice to have
- High School Degree required and AA preferred.
