About the role
- In addition, the PrEP Case Manager will provide HIV and STI risk reduction strategies, as well as medication adherence assistance.
- Job Duties and Responsibilities: • Conduct thorough screenings and intakes to assess clients' readiness for PrEP/PEP.
- Identify potential barriers to adherence, such as financial, housing, mental health, or substance use issues.
Responsibilities
- Maintain accurate and confidential client records in compliance with HIPAA and other standards.
- Develop and implement personalized care plans that address each client's unique needs, goals, and challenges.
- Develop collaborative relationships with community agencies to facilitate PrEP services for negative individuals and for their newly diagnosed clients to get connected to care.
- Access to information is based on a "need to know" and is the minimum necessary to properly perform assigned duties.
- The Case Manager will conduct screenings and intakes to determine program eligibility, in addition to developing individualized care plans.
- The PrEP Case Manager will have extensive client interaction and will collaborate with community partners in recruitment efforts to identify patients who are appropriate for PrEP services.
Requirements
- Bachelor's degree in social work, Human Services, Psychology, or related fields required.
- Minimum of 1 year of work experience in case management services.
Nice to have
- Working knowledge in the field of HIV and STI's, familiarity with community resources, the Harm Reduction model, ability to work in a fast paced, interdisciplinary team setting strongly preferred.
Compensation
- $70,068.00 - $108,605.00
