About the role

The Case Manager, RN is an integral team member of the FreedomCare Licensed Home Care team.
They assess our patients' current service needs and reassess needs for service upon significant change in health status.

Bachelor's Degree (BSN) and current NYS License or compact License that is in good standing (OR) associate's degree in nursing with relevant homecare clinical experience.
Minimum of 1-2 years of Nursing experience with a concentration in the homecare agency space.
Experience in geriatric care or working with individuals with disabilities.
Ability to travel within the 5 boroughs, as needed.
Strong interpersonal and communication skills.
Proficient in Microsoft Office Suite (Word, Excel, Outlook, etc.).
Excellent verbal and written communication, organizational and time management skills.

Train and supervise non-skilled caregivers in providing personal care, companionship, and assistance with daily living activities.
Monitor clients' overall well-being and report any changes to the appropriate healthcare professionals.
Educate clients and their families on effective care strategies, safety measures, and daily living skills.
Ensure compliance with agency policies, state regulations, and best practices in non-skilled care.
Act as a liaison between clients, families, and caregivers to facilitate communication and enhance the quality of care.
Participate in ongoing training and professional development to stay current with best practices in home care.
