About the role
- Act as a liaison between inpatient and outpatient providers within CCF and other facilities (e.g., rehabilitation facilities, access centers) and hemodialysis center staff.
- Facilitate communication between patients, family members/caregivers, hemodialysis facilities, Cleveland Clinic Florida (CCF) nephrologists, and other CCF departments.
Responsibilities
- Assess, evaluate, treat, and manage pre and post kidney transplant patients.
Requirements
- Typical physical demands include the ability to walk and stand for long periods of time.
- Follows standard precautions using personal protective equipment as required.
- Must be able to function independently to the full scope of licensure.
- Advanced Practice Provider - Certified Nurse Practitioner or Certified Physician's Assistant.
- Must possess excellent organizational, interpersonal, and communication skills.
- Ability to work in a fast-paced environment and meet deadlines.
- Must be flexible with schedule.
- Demonstrated independence in critical thinking.
- Nephrology and hemodialysis experience preferred.
- Kidney transplant experience beneficial.
- Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs.
- Additional Responsibilities:
- Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Works in collaboration with the health care team.

- You will be required to comply with this program, which will include obtaining an influenza vaccination on an annual basis or obtaining an approved exemption.
