About the role

Responsibilities
- Conduct home visits to assess and create personalized care plans.
- Train and support caregivers, ensuring they can meet their loved ones' needs.
- Maintain compliance with all relevant regulations and standards.

- You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).
- Assess client status upon initial home visit to identify needs in order to develop a comprehensive case management plan.
- Document in an accurate and timely manner.
- Maintain a clinical record for each morbidity patient.
Requirements
- Proficiency in common software and web-based applications including Microsoft Outlook, Excel, Word, PowerPoint, and Teams.
- Additional state-specific qualifications may be required.

- What You'll Bring Must hold an active Registered Nurse (RN) License in the state of employment Minimum two (2) years of experience in direct care of elders or adults with disabilities Ability to travel (usually by automobile) within assigned geographic area; valid driver's license and automobile insurance is required Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc) Experience with Minimum Data Set (MDS) assessment tool a plus.
- Completing the MDS-HC and other nursing assessments while coordinating all other applicable clinical assessments.
- Training, evaluating, and supervising caregivers.
