Coordinate team consult schedules and manage incoming consult requests for both inpatient and outpatient settings.
Coordinate with chaplain, social work, care management, and hospice/home care providers (GVNA) to ensure comprehensive patient support.
Collaborates with staff in other CHMG Palliative Care practice.
Perform data collection and data validation.
Performs job-specific requirements and other duties as assigned

Demonstrated an ability to work in a collaborative manner.
Experience Minimum of 5 years experience in acute care.
The employee is frequently required to do repetitive motion, hear, perform activities that require fine motor skills, reach, sit, speak, and walk.
The employee is occasionally required to bend, kneel, smell, squat, and stand.
Strong organizational, communication, and interpersonal skills.
Knowledge of palliative care principles, end-of-life issues, and healthcare systems preferred.
Proficiency in electronic health records and Microsoft Office Suite.
Demonstrates effective time management skills and attention to detail.

Specific vision abilities required by this job include color vision, depth perception, far vision, near vision, and peripheral vision.
Represent and introduce palliative care services to patients, families, and healthcare providers.
Review daily census, gather background information from referring providers, and medical records to triage consult requests and assist with determining priority.
