Responsibilities
- Coordinate team consult schedules and manage incoming consult requests for both inpatient and outpatient settings.
- Coordinate with chaplain, social work, care management, and hospice/home care providers (GVNA) to ensure comprehensive patient support.
- Collaborates with staff in other CHMG Palliative Care practice.
- Perform data collection and data validation.

Requirements
- Demonstrated an ability to work in a collaborative manner.
- Experience Minimum of 5 years experience in acute care.
- The employee is frequently required to do repetitive motion, hear, perform activities that require fine motor skills, reach, sit, speak, and walk.
- The employee is occasionally required to bend, kneel, smell, squat, and stand.
- Strong organizational, communication, and interpersonal skills.
- Knowledge of palliative care principles, end-of-life issues, and healthcare systems preferred.
- Proficiency in electronic health records and Microsoft Office Suite.
- Demonstrates effective time management skills and attention to detail.
Compensation
- $10k-$25k
