Responsibilities
- Manage a caseload of hospice patients as their primary RN, coordinating care from admission through end of life
- Collaborate with our interdisciplinary team - physician, social workers, chaplains, CNAs, LPNs, and volunteers to create a customized patient plan of care
- Provide emotional support and steady presence for families navigating anticipatory grief
- Participate in on-call shift rotation and perform ad-hoc patient visits as needed
- Document visits and coordinate medications, equipment, and supplies to keep care seamless
- Develops, implements and coordinates individualized, goal directed nursing care to patients using the nursing process as appropriate for RN's as applicable to the assigned area(s).
- Maintains nursing standards of care and responds to changing workloads and patient conditions by remaining calm, making objective assessments, prioritizing, and asking for assistance as appropriate.
- Transcribes and implements provider orders and treatments both verbal and written accurately and efficiently as noted by chart reviews and observation.
Requirements
- Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana required.
- Current Driver's License required.
- RQI Provider (BLS) required within 7 days of hire/transfer or before independent patient care, whichever occurs first.
- Must have knowledge of OSHA rules and regulations, established protocol for storing poisons, narcotics, acids, caustics, and flammable items
- Ability to act with integrity, kindness, and exhibit empathy.
- Minimum of two (2) years' experience in a similar position within a medical setting (e.g., physician office, clinic, or hospital) preferred. Experience in assigned area(s) preferred.
- Driving record for the previous thirty-six (36) months must not reflect any major driving offenses (i.e
- DUI, convicted of a felony involving a motor vehicle, suspended license, three (3) or more speeding tickets or other moving violations, etc.).
- In addition, personal vehicles used for LH business:
- State law minimum liability coverage limits required.
- Possess knowledge of professional nursing theory, medical terminology, and principles of professional nursing practice to administer and evaluate patient care.
- Understanding the confidentiality laws that govern the provider-patient relationship, including but not limited to, HIPAA guidelines.
- Knowledge of equipment, supplies, and instruments used in a medical office, simple routine clinical laboratory methods, universal blood and body fluid precautions.
