About the role

Requirements

  • Help families and caregivers complete the steps required to begin receiving services and support.
  • Manage a field-based schedule and execute visits with a high degree of ownership.
  • Guide clients and caregivers through required activation steps, forms, acknowledgments, and program documentation.
  • Success in this position requires strong execution, accurate documentation, clear communication, and the ability to work independently while traveling regularly.

  • Valid driver's license and automobile insurance, as regular travel within Ohio is required.
  • Strong organizational and time-management skills.

  • Basic proficiency with laptops, tablets, mobile applications, web-based systems, email, and Microsoft Office.
  • Experience working in Structure Family Caregiving (SFC) or a similar setting preferred.
  • Certifications in gerontology or case management preferred.

  • This is a field-based role focused on completing new-client activation visits, collecting required documentation, conducting BCI/FBI fingerprint scans, and ensuring each applicant and family successfully completes the onboarding process.
  • This role does not manage an ongoing client caseload or provide continuing care management after activation.
