About the role
- You will work collaboratively with an interdisciplinary team to ensure patients receive the highest quality end-of-life care.
- Administer medications and treatments as prescribed
- Participate in on-call rotation as assigned
Responsibilities
- Complete comprehensive patient assessments and develop individualized plans of care
- Monitor patient condition and manage symptoms in accordance with physician orders and hospice standards
- Collaborate with physicians, social workers, chaplains, aides, and other team members
- Maintain accurate and timely clinical documentation
Requirements
- Minimum of one year of nursing experience required
- Ability to travel throughout Fannin and Grayson Counties
