About the role
- We are seeking a compassionate and dedicated Physical Therapist (PT) to join our Home Health team.
- The Physical Therapist provides skilled therapy services in patients' homes under physician orders, helping individuals improve mobility, reduce pain, restore function, and maximize independence.
Responsibilities
- Perform comprehensive evaluations, including OASIS assessments, and develop individualized plans of care.
- Deliver evidence-based interventions to improve strength, balance, mobility, endurance, and functional independence.
- Maintain compliance with Medicare, federal, state, and company documentation standards.
- Perform other duties as assigned.

Requirements
- Minimum of one (1) year of experience as a Physical Therapist in an acute care setting required.
- Ability to prioritize workload, meet documentation deadlines, and work independently.

- Complete all required clinical documentation accurately and within agency timeframes, including Start of Care, evaluation, recertification, routine visit, and discharge documentation.
- Home health experience preferred.
- Current, unrestricted PT license in the state of employment.
- Strong clinical assessment, critical thinking, and patient education skills.
- Excellent communication and collaboration skills with patients, families, physicians, and healthcare professionals.
- Valid driver's license, reliable transportation, and current auto insurance.
- Education & Licensure
- Graduate of an accredited Physical Therapy program.
- Current license to practice as a Physical Therapist (PT) in the state of employment.
- Physical Requirements
- Frequently walk, stand, bend, reach, and lift up to 50 pounds with or without assistance.
