About the role

  • Wider Circle is looking for a Care Navigator to join our team in Care Delivery Operations.
  • In this entry-level role, you'll talk with patients by phone (and sometimes in person) to complete health assessments and intake appointments.
  • This role is a strong first step toward becoming a Care Advocate, with a clear path to grow into that position.

Responsibilities

  • Build quick rapport so patients feel comfortable sharing accurate information during each 15-20 minute call.
  • To do this job well, Care Navigators need to be able to: Sit, stand, and walk regularly to talk with members and manage day-to-day tasks.
  • Builds trust quickly over the phone.

Requirements

  • Space out your appointments so you have time to review your notes carefully..
  • High school diploma or equivalent required.

Nice to have

  • Fluency in English and Spanish (or another language) is strongly preferred.
  • Bilingual: Fluency in English and Spanish (or another language) is strongly preferred.
