Responsibilities

  • Manage a panel of complex pediatric patients: Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination
  • Lead a multidisciplinary team: Partner with nurses, pharmacists, dietitians, and social workers to provide wraparound care in a virtual-first model
  • Deliver high-quality virtual care: Conduct virtual enrollment visits, acute escalations, sick visits, post-hospital follow-ups, and in-home virtual assessments with support from our Mobile Integrated Care team (paramedics).
  • Perform other duties as assigned.

Requirements

  • Enrollment in applicable state Medicaid required.

Compensation

  • The role offers a base salary range of $210,000 - $225,000 in addition to annual bonus incentive, competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable).

  • In addition to delivering exceptional virtual-first care, a significant portion of this role will focus on external engagement, brand advocacy, and community outreach.
