Implement care plan interventions with attention to both clinical outcomes and participant engagement
Support the management of participant caseloads in collaboration with RN Case Managers

Through the Program of All-Inclusive Care for the Elderly (PACE), we provide comprehensive medical care along with support for daily needs such as meals, transportation, and in ‑ home assistance.
Administer medications and injections, monitor vital signs, and observe participant health status
Perform assessments and document participant conditions, responses to treatments, and changes in health status
