- While this role is remote, candidates must reside and licensed in New Jersey or North Carolina.

Responsibilities
- Manage a panel of complex pediatric patients - Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination.
- Deliver high-quality virtual care - Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments supported by our Mobile Integrated Care team.
- Your passion for creating equitable, accessible care aligns with our mission to ensure every child receives the support they deserve.
Requirements
- You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification.
Nice to have
- Active NJ or NC licensure is required, and an additional license in FL, NY, GA or DC is preferred
- Virtual care experience a plus.

- Minimum 5 years' experience with a pediatric population required, Community Based Health or Complex Care strongly preferred.
- Experience and comfort with children with complex medical needs
- You're driven by a commitment to transform pediatric healthcare, especially for children with special health care needs.
- Experience with Medicaid and/or Community health preferred
