While this role is remote, candidates must reside and licensed in New Jersey or North Carolina.
Our goal is to increase the number of safe days at home by ensuring kids and families get what they need when they need it-reducing unnecessary medical visits and hospitalizations.

Manage a panel of complex pediatric patients - Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination.
Deliver high-quality virtual care - Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments supported by our Mobile Integrated Care team.
Your passion for creating equitable, accessible care aligns with our mission to ensure every child receives the support they deserve.

You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification.

Active NJ or NC licensure is required, and an additional license in FL, NY, GA or DC is preferred
Virtual care experience a plus.

Minimum 5 years' experience with a pediatric population required, Community Based Health or Complex Care strongly preferred.
Experience and comfort with children with complex medical needs

Experience with Medicaid and/or Community health preferred
