About the role
- The Clinical Coordinator is a multifunctional position with aspect of care management, risk assessment and quality assurance analysis and implementation.
- Oversee post-discharge activities in order to ensure continuity of care as patient's transition from inpatient to community.
- Identify patients that will benefit from case management.
Requirements
- Experience with EHR a must, as well as basic computer skills.
- Knowledge of professional nursing theory and practice to give and evaluate patient care.
- Knowledge of organizational policies, regulations and procedures to administer patient care.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Ability to assess the specific age-related needs of patients.
- Ability to react calmly and effectively in emergency situations.

Skills
- Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.
- Skill in preparing and maintaining records, writing reports, and responding to correspondence.
- Skill in establishing and maintaining effective working relationships with patients, medical staff and the public.
Compensation
- Starting pay range varies based on experience up to 15 years.
