About the role

  • The Clinical Coordinator is a multifunctional position with aspect of care management, risk assessment and quality assurance analysis and implementation.
  • Oversee post-discharge activities in order to ensure continuity of care as patient's transition from inpatient to community.
  • Identify patients that will benefit from case management.

Requirements

  • Experience with EHR a must, as well as basic computer skills.
  • Knowledge of professional nursing theory and practice to give and evaluate patient care.
  • Knowledge of organizational policies, regulations and procedures to administer patient care.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Ability to assess the specific age-related needs of patients.
  • Ability to react calmly and effectively in emergency situations.

Skills

  • Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.

Compensation

  • Starting pay range varies based on experience up to 15 years.
