About the role
- Our interdisciplinary care team includes nurses, registered dietitians, social workers, and care coordinators who work together to improve health outcomes-all from the comfort of our patients' homes.
- Karoo is seeking to add an important care team member, the Patient Engagement Specialist.
- This individual will be responsible for new patient acquisition and play a critical role in driving growth within the organization.
Responsibilities
- Build trusting relationships with patients and caregivers to encourage enrollment Conduct in-person outreach in hospitals, provider offices, community settings, and patient homes as assigned.

- We partner with health plans and providers to deliver comprehensive support for individuals living with heart disease and other chronic conditions.
- The PES will conduct outreach by phone and in person, educating patients about the benefits of the program, answering questions, addressing concerns, and guiding them through the enrollment process.
- Contact eligible members by phone to introduce Karoo Health's cardiac care management program and conduct enrollment onboarding sessions.
- Pre-screen patients for eligibility, e ducate patients about the benefits of participating in the program, and answer questions.
