About the role

The TOC pharmacist is responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes upon hospital admission.
Participates in and contributes to educational programs, committees, and clinical research.
The Medication Safety Clinical Pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research.

Maintains and updates a patient profile with demographics (e.g. height and weight), diagnosis, allergies, and current medications.
And everyone, in their own unique way, plays an important part, every day.

Doctor of Pharmacy (Pharm.D) degree
OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience.
Minimum of 1 year of direct patient care experience
Medication Therapy Management certification or equivalent.
ASHP Accredited Residency in Pharmacy Practice (PGY1).
ASHP Accredited PGY2 Specialty Pharmacy Residency.
